Villa Family Clinic, PC is required by law to maintain the privacy of your protected health information (PHI), to provide you with notice of our legal duties and privacy practices, and to notify you following a breach of unsecured PHI.

- Request a copy of your medical record, and request corrections to it.
- Request restrictions on how your information is used or disclosed.
- Request confidential communications (for example, contacting you only at a certain number).
- Receive a list of certain disclosures we have made of your information.

- File a complaint — with us directly, or with the U.S. Department of Health and Human Services — without fear of retaliation.
